On KB, KV, CZ we should read the vram width from integrated system
table, if we can. The NOOFCHAN in MC_SHARED_CHMAP is not accurate.
With this change we can enable two 4k displays on CZ again. This use
case was broken sometime in January when we started looking at
vram_width for bandwidth calculations instead of hardcoding this value.

The contextID field (formerly known as src_data) of the IH
vector stores client specific information about an interrupt.
It was expanded from 32 bits to 128 on newer asics. Expand the
src_id field to handle this.

We don't need to use the PCI BAR on APUs. This allows us to access
the full VRAM directly without being limited by the BAR size.

This breaks on systems with huge amounts of system memory as we do not have
enough vram to store the page tables. Additionally, this is less of an issue
with the recent gtt manager changes.

It can be the case that upon GPU page faults we start trashing
the logs, and so let us ratelimit here to avoid that.
V2. Fix issue where calling dev_err_ratelimited separately for
each line means that some lines corresponding to a single
VM fault may or may not appear depending on the rate.

This makes it easier to replace specific IP blocks on
asics for handling virtual_dce, DAL, etc. and for building
IP lists for hw or tables. This also stored the status
information in the same structure.

In some cases the amount of vram may be less than the BAR size,
if so, limit visible vram to the amount of actual vram, not the
BAR size.

The VM default page (used when a VM translation fails) is allocated in
system memory. The VM is misconfigured to interpret the physical address
as referencing a VRAM physical page.
Route default page accesses to system memory.

Track the type of vram on the board and provide a query for it.
User mode drivers and tools want this information for determining
bandwidth information and form informational purposes.
